ALL WQ TEAMS must print the following form (link below), fill it out completely and return it via email to mvapa183@gmail.com, NO LATER THAN May 31st. 

EVERY PLAYER ON YOUR SPRING SESSION ROSTER MUST SIGN AND FILL IT OUT! Remember that shooters must have 10 matches played in the last two years by the end of the Spring session in order to be eligible to compete in WQs-no exceptions. THIS IS A NATIONAL RULE THAT WE HAVE POSTED MANY, MANY TIMES.

Players MUST certify at their highest SL since qualifying for the WQ(including their current SL, if it is their highest). Failure to do so could result in the disqualification of your team.  

In addition, captains, if you have a member or members of your SPRING roster that were NOT on your team when you earned WQ qualification, please note that in your email.

National is not taking any of this lightly and failure to follow proper procedures could result in a loss of qualification for your team.
