2024 World Qualifier - Team Certification Form

Attention all World Qualifier teams:

Only 8-Ball & 9-Ball teams (Not Destination Division or Cash Tournament)

Each team MUST have every player who plans on competing at the World Qualifier sign this form to be handed in at least 1 HOUR BEFORE YOUR FIRST ROUND of the event that you are competing in. You can print it yourself usingn the link below (making sure to shrink it so it all fits) OR you can pick one up at select locations starting 5/8: 3rd Street Pub, Bulldogs Cellar, Lady T's, Rudy's, Suzy's Pizza, Corrals. They will be in a folder labeled "2024 World Qualifier Certification Forms" behind the bars.

Read the entire form so that players are verifying the correct handicap. Players who do not do this risk not being able to play or if they play at the tournament with the wrong handicap they could face forfeit or team DQ. READ THE FORM. IT EXPLAINS WHICH HANDICAP YOU ARE SUPPOSED TO REPORT AT ON THE FRONT TOP PORTION!

Teams must turn their entire signed form in before they play their first match so I recommend sending it with someone who will be there early.

There will also be forms on site for teams who could not get them filled out. I can't stress this enough if you wait, ALL PLAYERS PLAYING MUST BE PRESENT TO SIGN BEFORE YOUR FIRST ROUND BEGINS! Working late, caught in traffic, flat tire, not matter the reason they will not be able to play if not signed before the round begins.

  • IT IS ALWAYS YOUR SPRING SESSION ROSTER that is invited to the World Qualifier. All players who got minimum plays (6) in Spring Session and have at least 10 lifetime plays in the format by END OF SPRING SESSION will be on World Qualifier roster.
  • All players playing at the World Qualifier must be on a Summer Session roster at the time of WQ in the same format or they will be removed from the WQ roster.. (Example... if playing 8-Ball WQ you do not need to be on the team that you qualified with. It can be any Summer Session 8-Ball team.)
