Spring World Qualifier Teams - Complete and return your Team Certification Form

8-Ball Cert Forms are due in the office by April 28
9-Ball Cert Forms are due in the office by May 19

Memberships and Team Envelopes

ALL PLAYERS must have a paid membership to play, if a team plays an unpaid player the team will be charged for the membership fee and lose bonus points for that week.
Membership payments can be made in the mobile app, at league.poolplayers.com, or in cash during league play.
All information must be completed on the team envelope and fees included for bonus points to be awarded.
