If you win your Division you get to choose your Team Award!!!

STANDARD

  • The standard award for Division Champions includes the following:
    • Team Plaque with Players Names
    • Individual Player Trophy for each member of the team.
    • Division Champions Patch
    • Players must have played at least 4 matches during the session.· In order to receive this award you do not need to send this form back

ALTERNATE AWARD 1 – T-Shirts

  • The team may select T-Shirts as an award. (Will have Division Champions on the T-Shirt)
    • Team Plaque with Players Names and Division Champions Patch will still be included in this option.
  • In order to qualify for this award the following conditions must be met
    • Players must have played at least 4 matches during the session.

ALTERNATE AWARD 2 – 2 Weeks Free

  • The team may select Team Weekly Fee as an award.
  • Team will receive 2 Week’s Team Fees covered.
  • · Only valid for the Team in the Format they won. (8-Ball/9-Ball)
  • · $100 Value
  • · Non-Transferrable.
  • · Team Captain / Co-Captain and at least two other players must sign this form. (See back)
  • · Any divergence of these requirements will result in the STANDARD award option for the team.
  • · Team Plaque with Players Names and Division Champions Patch will still be included in this option.
