1️⃣ Combined Skill Level: 12 max

2️⃣ Eligibility: Both players must have 10 lifetime 8-Ball scores by Nov 17, 2025 (within the past 2 years)

3️⃣ Team Entries Only: Both players must be listed at registration — single entries will be deleted

4️⃣ Save Proof: Screenshot your confirmation number once you submit your form

5️⃣ Payment: Do NOT pay until the League Office contacts you to confirm your team’s spot

6️⃣ Once 24 slots are filled, we’ll open a wait list
