States Qualified Captains recieved 2 emails with the Certification Form needed to be printed, filled out, signed by the players on the team and scanned and emailed back to the office by THIS FRIDAY JUNE 13th! Bring the physical copy with you to States!

If you do not have it completed and/or you come in asking for a blank one, you will have to deal with Pauline!

If you are a captain of a States Qualified team and you didn't recieve anything from the office - contact Pauline ASAP!
